McCarthy made his feature film directorial debut in 2020 with Caveat, which he also wrote and edited. That movie stars Johnny French as a man who accepts a job looking after a psychologically disturbed woman in a house on a secluded island. Currently available to stream on Shudder, Caveat received positive reviews from critics, as it currently has an 81 percent Tomatometer score on Rotten Tomatoes.
Following that, McCarthy then made Oddity in 2024. Starring Carolyn Bracken as a blind medium who is grieving the death of her twin sister, Oddity premiered at South by Southwest, where it won the Audience Award in the Midnight section of the festival.

The film is produced by Roy Lee and Steven Schneider for Spooky Pictures, Derek Dauchy for Image Nation, and Ruth Treacy, Julianne Forde, and Mairtín de Barra for Tailored Film. Executive producers include Cweature Features’ Ken Kao and Josh Rosenbaum, as well as Ben Ross, Dan Kagan, Rami Yasin, Andrew Childs, and Team Thrives’ Bryan Meng and Dr. Terence Chan.
